Campers & Sponsors must do the following:

• Register & Pay online
• Print Permission form (give completed form to youth leader)
• Print insurance card (give completed form to youth leader)

REGISTRATION

Registration – The registration process is as follows:

• Step One – Fill out online (available at nwilnaz.org)
• Step Two – Submit a copy of both sides of your insurance card to the Camp Coordinator at your local church
• Step Three – Submit a signed copy of the NWIL Permission Form to your youth leader
• Step Four – campers medication must be in original bottle and given to the camp nurse upon arrival

Counselors – Each church must send one counselor for every six campers you are sending. For SOUL SURVIVOR Camp, assistant counselors must be 18-21 years old (must have completed first year of college) and lead counselors must be 22 or older. (Read policy attached) Counselors must complete Ministry Safe Training. You will receive an email from the district office with the training link.
